Registration Form
You can print out the registration form and have it filled out for us the first time we come to your house, or we can provide the form for you and wait for you to fill it out when we arrive. It asks for your name, address, phone number, email, ID# or validation phone number, date of recommendation, and type of ID.
Show the medical cannabis ID card or doctor's recommendation to us the first time we provide your medicine. After that, we'll just need to see valid picture ID for further deliveries. The doctor's recommendation is good for one year. Thank you for your cooperation.
